Can you get a doctor's note for anxiety online? Yes. If anxiety is affecting your ability to work, a US-licensed physician can review your situation through a quick online form and, where appropriate, issue a short absence note, typically the same day, with no clinic or video visit in most states.

When you might need a note for anxiety

There are days when anxiety makes it genuinely hard to function at work, and a short break to regroup is reasonable. Many employers ask for documentation when you take a sick day. An online review is a discreet, legitimate way to get a brief absence note so you can step back and look after yourself. It is not a substitute for ongoing mental-health care.

What the note includes

Your note confirms that you reported feeling unwell and were advised to take a short period of rest for the relevant dates. It is intentionally general and does not diagnose, treat, or prescribe. It is a simple, private absence note from a licensed doctor, watermarked, and it can be verified with your permission.

We focus on legitimate, low-risk notes. We don’t issue FMLA, disability, or fitness-for-duty paperwork, and we never diagnose, treat, or prescribe online.

How it works

  1. Tell us what’s going on

Answer a short, adaptive 2-minute form about your symptoms — not a static tick-box questionnaire.

  2. A clinician licensed in your state will review it

At launch, a licensed clinician will review your written answers, may ask a follow-up question, and can decline if a note isn’t appropriate.

  3. Get your note by email

At launch, notes will arrive as a PDF with a verification code. Turnaround targets will be published before we open.

If your symptoms are severe — trouble breathing, chest pain, confusion, a very high fever, or you feel you’re getting worse — seek in-person or emergency care. An online absence note is not a substitute for treatment.

Everyday anxiety before a hard conversation or a deadline usually passes within a day, and it can even help you focus. An anxiety disorder is different: the fear and worry do not go away on their own, and they can get worse over time. According to the National Institute of Mental Health, anxiety disorders interfere with daily activities such as job performance, schoolwork, and relationships. A short absence note through this service reflects that distinction. It covers a brief period of rest for the dates you report. It is not a diagnosis, a treatment plan, or an estimate of how long an anxiety disorder itself will last, since that depends on the person and the type of anxiety involved.

The causes of anxiety disorders are not fully understood. Genetics, brain chemistry, ongoing stress, and your environment can all play a role, and some physical conditions, such as thyroid problems, can produce similar symptoms. That is one reason a single absence note is not meant to substitute for a proper evaluation if anxiety keeps recurring.

When anxiety needs more than a day off

Generalized anxiety disorder involves excessive worry about ordinary things, such as health, money, work, or family, on most days for six months or more. Panic disorder involves sudden attacks of intense fear with no real danger, usually lasting several minutes or more. Both can come with physical symptoms: a pounding or rapid heartbeat, dizziness, shortness of breath, or aches that do not have another explanation. If your anxiety fits that pattern rather than a single rough week, an absence note will not treat it. Psychotherapy, medication, or both are the main treatments, and your regular doctor or a mental health professional can start that process.

If you are in crisis or thinking about harming yourself, an absence note will not help you in the moment. Call or text 988, the 988 Suicide & Crisis Lifeline, for free, confidential support, any time of day. Chat support is also available online, and the line has options for Spanish speakers and people who are Deaf or hard of hearing.

What to tell your employer, and what stays private

Sick-leave policies differ by employer, and some ask for documentation for any absence, including a mental health day. You do not have to name anxiety, or any diagnosis, on the note itself. A general absence note confirming you were advised to rest is usually enough. Because the note comes from a licensed physician, your employer can confirm it is genuine, but only with your permission: your medical information stays private unless you choose to share it.

If you are telling a manager directly, something simple works, such as “I’m dealing with a health issue and need to take today off.” This service issues a short absence note only. It does not complete FMLA paperwork, disability forms, or workplace accommodation requests; for those, your employer’s HR or benefits office is the right contact, and a mental health professional can help document an ongoing condition if one is needed.

Frequently asked questions

Is this a diagnosis or a treatment plan?
No. This is a short absence note, not a diagnosis, therapy, or medication. If you need ongoing support, please connect with a mental-health professional or your regular doctor.
Will my note say "anxiety" on it?
It doesn't have to. Absence notes can simply confirm you were advised to rest, keeping the reason private from your employer.
What if I'm in crisis or thinking about harming myself?
An absence note isn't emergency help. If you're in crisis, call or text 988 (the 988 Suicide & Crisis Lifeline) any time, day or night, or call 911. You deserve real-time support.
Will my employer accept the note?
A note signed by a licensed physician is a legitimate medical document. Employer policies on what documentation they require vary. More →
Can this service help with FMLA paperwork or disability accommodations for anxiety?
No. This service issues a short absence note only. FMLA paperwork, disability forms, and workplace accommodation requests go through your employer's HR or benefits office, or your regular doctor.
