How schools generally decide when a child can return

CDC guidance for K through 12 schools sets out general symptom-based criteria that many districts use as a starting point, though local policy has the final say. Under that guidance, a child can generally return once a fever has been gone for at least 24 hours without fever-reducing medicine, vomiting has resolved overnight with food and liquids staying down the next morning, and diarrhea has improved to no more than two bowel movements above the child’s normal count in a day. For respiratory symptoms such as a cough or runny nose, the same 24-hour rule applies: symptoms should be getting better overall, and any fever gone, for a full day before returning. These are general benchmarks, not a fixed calendar, since illnesses run different courses in different kids.

What a typical illness-related absence looks like

Most of the common reasons a student misses school, colds, seasonal viruses, a stomach bug, are short and self-limited. CDC’s school guidance is explicit that many common childhood illnesses can be managed at home without a clinic visit, and that requiring a provider visit solely to obtain a note can be an unnecessary burden on families, since it can mean extra time off work or an added copay for a visit that wasn’t otherwise needed. That’s the gap an online review is meant to fill: getting the school the documentation it asks for without turning a manageable illness into a second appointment.

When a school absence needs more than rest

A short absence note is meant for the ordinary case of a student who’s unwell for a day or a few days and needs rest. It isn’t the right tool for a condition that needs an actual exam or treatment plan, a new or worsening symptom that hasn’t been evaluated, or a fever that lasts several days. If a child has a high or persistent fever, a rash with fever, trouble breathing, or symptoms that improve and then come back, that calls for an in-person evaluation rather than a same-day note. Schools may also have separate, illness-specific policies (for conditions like strep throat) that call for their own return criteria, and those take precedence over general guidance.

What to tell the school office

A doctor’s note for a school absence doesn’t need to spell out a diagnosis to be valid. Under this service, the note confirms that the student was seen through an online review, reported being unwell, and was advised to rest for specific dates; a parent or guardian handles the review and consent for a minor. If the school’s attendance policy requires something more specific, such as a note for absences beyond a set number of days or one addressed to a particular office, it’s worth checking that policy before the absence rather than after, since excused-absence rules are set at the district level and vary from school to school.
